The Multipurpose Senior Services Program, or MSSP as we call it, is a Federal Waiver program serving frail elders 65 and older, who are insured through Medi-Cal with no share of cost and who are certified for nursing home placement. The purpose of this program, like all of our programs at RSNC, is to promote dignity and independence. This is achieved through a team of social workers and nurses who provide care management services for our frailest and most vulnerable Contra Costa residents. This team provides a thorough assessment of each individual and creates a care plan with the goal of preventing nursing home placement. Services include assisting clients with issues such as medical monitoring, advocating for in-home care services, purchasing equipment to ensure safety in the home, and much, much more.
