Services Offered to Frail Seniors & People with Disabilities

Adult Day Health Care was developed in the 1970's with two goals in mind. One is to achieve/maintain the optimal level of functioning for adults with disabilities, dementia and/or those who are frail. The second goal is to offer respite to their caregivers.

Our Adult Day Care programs offer a caring, supportive, and social environment. Each participant has an individualized plan of care created by a team of nurturing professionals. This plan defines how daily activities can be tailored to fit an individual's specific needs, and is designed to increase vitality and well-being while maximizing physical and mental abilities, in an interactive and social setting.

This program is a great alternative for families who are not able to provide full-time care and support for their loved ones, yet wish to have them remain living at home.

Features and benefits of the program:
- Individualized care plans based on the needs of each individual
- A safe, supportive, and nurturing environment
- In depth medical case management from specialized, trained nurses, social workers, physical, speech and occupational therapists
- Nursing treatment and psychological services as needed
- Daily hot, nutritional noon meal
- Social interaction, friendship and connections with staff, volunteers, and other program participants
- Daily social, recreational, and educational activities, arts and crafts, music, exercise, discussions, games, dancing, gardening, current events and more!

The program also provides respite to caregivers who may be struggling with the daily demands of caring for an elder, as well as providing information and referral services for their unique caregiving situation.
